Registration and Enrollment

You can register your child online.  Once you have completed the registration,  you will be contacted and notified as to whether your child is on the waiting list or there is space available for immediate enrollment.  You may also plan a tour of the center at that time.  Once a child is enrolled with a planned start date, the administrator will set up a time to review policies and procedures of Creative Academy and complete enrollment paperwork.  At that time, there will be a $35 enrollment fee charge.

Please return the child application and child medical history form to Creative Academy at 1109 Royall Ave.  or send to brooke.king@skillcreations.com.  Please attach copies of your child's current physical and immunization record.  These documents are required prior to a child entering care.        Click HERE to download the application.
